ALASKA

UI CLAIM REQUIREMENTS

Your name, address, social security number and other personal details such as telephone number, email address etc.

  • Dependents name, date of birth and their social security number.
  • Details of your previous employer. For each employer, you need to provide
    1. Name and address
    2. Telephone number
    3. Start and end dates of employment
  • If you had out-of-state employment in the past 18 months, you also need to provide the employment details.
  • Reason you lost the job – it has to be involuntary to be eligible to receive benefits.
  • Your job skills summary
  • If you are not an US citizen, keep your alien registration number and work authorization. However if you are on a work visa such as H1-B, immigration experts do not advise claiming unemployment insurance.

ARIZONA

UI CLAIM REQUIREMENTS

  • Your Social Security Number.
  • Arizona Driver’s License or Arizona State Issued ID.
  • Your mailing address, city, state and ZIP code.
  • The county where you reside (if filing your new claim on the Internet).
  • The names, addresses, and phone numbers of all your employers for the last 18 months.
  • The last day you worked immediately prior to filing your UI claim.
  • If you received severance, vacation, holiday or payment for unused sick pay: give the amount (before deductions) and the date it was paid to you.
  • The name and local number of your union hall, if applicable.
  • Alien Registration Number, if applicable.
  • Copy # 4 of your DD Form 214 if released from the military in the last 18 months.

FOOD STAMPS ELIGIBILITY

If you qualify for emergency Nutrition Assistance you will be able to receive benefits within 7 days from the date of application. You may be eligible for emergency Nutrition Assistance benefits if you are not currently receiving Nutrition Assistance benefits and your household situation is one or more of the following: 

  • Your cash, savings and checking accounts are not more than $100 and your income is less than $150, before deductions, in the month of application.
  • Your monthly housing and utility expenses are more than your monthly income before deductions plus your cash, savings and checking accounts.
  •  You or a member of your household is a migrant or seasonal farm worker and your cash, savings and checking accounts are not more than $100.

 IMPORTANT DOCMENTS:  To be approved for emergency Nutrition Assistance you must provide:

  • proof of your identity (driver’s license, birth certificate, immigration card, pay stub). All other required proof documents may be postponed, but must be provided to continue receiving benefits. Fax or upload proof of identity with your application.
  • Identification (I.D.) for applicant for Cash Assistance (CA) and Nutrition Assistance (NA).
  • Proof of citizenship and identity for everyone who is applying for AHCCCS Health Insurance (Medical Assistance or MA), Cash Assistance (CA), and Nutrition Assitance (NA).
  • Alien Registration Cards, if there are non-U.S. citizens in your household.
  • Social Security numbers for everyone, or proof that a Social Security number has been applied for.
  • Birth certificates for everyone.
  • Name, address & daytime phone number of landlord or neighbor.
  • A statement verifying your address and the names of everyone living with you. The statement must be made by a non-relative who doesn’t live with you.  It must be signed, dated and include that person’s address and telephone number.
  • Proof of ALL money your household received from any source last month and this month. 
  • Proof that your employment ended and last date paid.
  • Bank or credit union (savings or checking) complete statement for the most recent month. 
  • Proof of savings bonds, securities, retirement plans and life insurance.
  • Proof of pregnancy, including the estimated date of delivery.
  • Proof of rent/mortgage and utility bills (electric, water, gas, etc.) for the most recent month.
  • Proof of child care expenses for the most recent month.
  • Proof of all medical expenses for those who are age 60 or older or receive disability benefits if they are applying for Nutrition Assistance.

If you have a face-to-face interview, please bring verification with you of your income and expenses. If you cannot get all the information together by your interview date, still come for the interview because you will have additional time to provide this information. If you need assistance in obtaining this information, please discuss this with your CalFresh worker at the time of the interview. The following are examples of what to bring:

COLORADO

UI CLAIM REQUIREMENTS

In order to receive UI benefits from Colorado, you must affirm that you are a citizen or a legal, permanent resident of the United States or otherwise legally reside in the United States in accordance with federal law. You must possess and provide information regarding one of the following forms of identification (ID):

  • A valid Colorado driver’s license or a valid Colorado ID card.
  • A United States military ID card or a military dependent ID card.
  • A United States Coast Guard Merchant Mariner card.
  • A Native American tribal document.

NOTE: Unemployed workers living outside Colorado must provide information proving that they have a valid driver’s license or ID card issued by another state or by Canada.

You must affirm your citizenship or legal presence in the United States by signing and returning Form UIB-1, Claim for Unemployment Insurance Benefits. Also, please provide information about your proof of ID on the same form. The form is mailed to you after you file a new UI claim, file an additional UI claim, or reopen an existing UI claim. You must provide this information only once during each claim year. The form must be completed, signed, and received by the UI Program within the timeframe specified on the form or your claim for UI benefits will not be paid for the weeks prior to the week in which the form is received by the UI Program.

CONNECTICUT

UI CLAIM REQUIREMENTS

Do not delay in filing a claim if you do not have your separation packet which includes a pink slip. Your claim will be taken without it. Your claim is effective the Sunday of the week in which you first file for benefits. Ordinarily, you do not get paid for the weeks prior to the week you filed your initial claim.

  • Have your Social Security card and separation packet, if one was provided.
  • If you are separating from the military, have separation form DD214, Member-4.
  •  Federal employees should bring separation form SF-8 and a copy of their most recent pay stub.
  • If you are not a US citizen, you must have proof of legal alien status.
  • Do not delay filing a claim if you do not have these documents. Your claim can be filed without them. However, there may be a delay in payment until the document is received.

What happens in the interview?

 The caseworker asks questions about your household’s income, resources, rent or mortgage, and utility costs.  Certain households may also be asked about medical expenses, childcare and child support expenses.  Proof of your household situation is necessary, so if you have the following information, you may bring it with you:

Step 1. Fill out an application.

Anyone may fill out an application. Answer as many questions as you can. If you are applying for SNAP and can’t fill out all 8 pages of the application today, be sure to fill out this page and turn it in. But, then please fill out and turn in the rest of the application (pages 2-8) as soon as you can.

 Step 2. Return the application to us.

You can fax or mail your application to a local Department for Community Based Services (DCBS) office. Or, you can bring your application to a local DCBS office when it is open. When we get your application, an interview will be set up with you. You have the right to know soon whether you will get benefits. The date we get this page with your name, address and signature starts the time that we have to determine if you are eligible for food benefits. It is also the start date of food benefits for you if you are eligible for benefits.

 Step 3. Talk with us.

At your interview, you will need to show us:

  • Proof of who you are, such as your driver’s license, social security card or alien documentation;
  • Proof of who lives in your home, such as a lease or written statement;

FOOD STAMP REQUIREMENTS

For the Department of Social Services to accept your application, all you need to fill in is your name, address and signature. You can provide the rest of the documentation below at your interview.

  • Proof of Identity – This does not need to be a photo ID. If you do not have a document to prove your identity, you can bring along a friend, neighbor or family member who can verify your name and identity.
  •  Proof that you live in Maryland – Your driver’s license or mail addressed to your current residence will fulfill this requirement. Other options include a recent rent receipt, copy of your lease, mortgage payment, or your landlord’s name and phone number. Alternatively, you can bring a signed letter from a neighbor stating that you live at that address.*

  • Proof of Housing Expenses – Bring recent rent or mortgage payment receipts. Bring receipts of all recent utility bills: phone, gas, electric and water. Be sure that all of these documents have your address printed on them. 
  •  Proof of Immigration Status – You must provide proof of your immigration status. You should bring the immigration and naturalization papers for yourself or whomever you are applying for. Your papers are not required if you are applying only for children who were born in the United States. 
  • Proof of Child Care Payment – If you are legally obligated to pay child support to or for someone outside your household, you will be required to prove your legal obligation and the amount you are paying.
  • Proof of any Assets – A small number of applicants will be asked how muchmoney they have on hand or in savings or checking accounts. Unless you live with someone who has violated Food Stamp program rules in the past, you will not need to provide information on assets.

The Application Process:

  • You may request an application for SNAP benefits by phone, in person, by fax, or by mail from your county office.
  • Applications for SNAP benefits are available online.
  • One adult household member or authorized representative must sign the application for SNAP benefits under penalty of perjury.

  • Telephone number where we can contact you between 8 am and 5 pm Monday through Friday
  • Employer Registration number or Federal Employer Identification Number (FEIN) of your most recent employer (FEIN may be found on your W-2 forms), if you have it

  • Alien Registration card number (if you are not a U.S. Citizen)
  • If you are unable to print, have a pen and paper available to write down important information
  • Remember, if you are choosing to have direct deposit of your weekly benefits, you must have a check handy in order to enter your bank routing and checking account numbers

You will be required to establish a 4-digit Personal Identification Number (PIN) in order to complete your application and access information regarding your claim.

If you currently reside in Canada or your most recent employer has a Canadian address, you must file for unemployment insurance benefits by calling the Telephone Claims Center at 1-888-209-8124 (1-877-358-5306 for out of state residents).

  • Reason you lost the job – it has to be involuntary to be eligible to receive benefits.

  • If you are not an US citizen, keep your alien registration number and work authorization. However if you are on a work visa such as H1-B, immigration experts do not advise claiming unemployment insurance
